Next Steps for Counselling
with Family Physician Psychotherapy Specialist
Thank you for submitting your interest and intake for counselling with one of our Grace Health Centre Family Physician Psychotherapy Specialists.
A Secure Intake Form Request You will be receiving an email soon requesting you to complete our Consent & Getting to Know You intake form to the email address that you have indicated to us. This comprehensive form will be sent through our secure Ocean platform. Family Doctor Referral Request and Screening Consultation In the meantime, the required immediate next step is to obtain a referral letter and schedule a screening consultation.
Individuals with a family doctor:
Please coordinate with your family doctor to send a referral request to us for either Dr. Tat-Ying Wong or Dr. Alice Chen. Digital referral requests can be made through eReferral on Ocean Health Map https://oceanhealthmap.ca/ by searching for Grace Health Centre – Family Medicine & Counselling and Psychotherapy Clinic. Paper referral requests may apply to your family doctor. You may download this Referral Request Form and have your family doctor send in a completed copy to this email address therapy@gracehealthcentre.ca or fax to 416-633-8099 at their earliest convenience.
Individuals with no family doctor:
Once you have completed the “Consent & Getting to Know You” form, please call the Grace Health Centre office at 416-633-7337 or email therapy@gracehealthcentre.ca to arrange an in-person consultation visit with the associated family physician psychotherapy specialist (Dr. Tat-Ying Wong or Dr. Alice Chen) at our clinic location (2830 Keele St, Downsview, ON).
