Our Intake Process

As we take your confidence in us seriously, we have a thorough intake process to prepare you for counselling. Please read the following process carefully so that you can make effective use of our counselling resources.

EASY 3-STEP PROCESS

Choosing to work with Therapists Counselling Team

STEP 1: Fill in our Intake Forms (Form A – Counselling Request Form and Form B – Confidentiality and Electronic Communications Consent Form)

Form A allows you to select your preferences for therapy format (online or in-person), type (individual, family or couples), language (English, Mandarin or Cantonese) and a preferred therapist (each therapist profile is available in the ‘About Us’ section on the website detailing their education, experiences and modalities practiced)

STEP 2: Schedule a free 15 min consultation

Once we have received both your intake forms, our office will contact you within a few business days and inquire about potential dates and times for a free consultation. This interaction will determine if the therapist is the right fit for your needs and concerns.

STEP 3: Book regular therapy sessions

Give us a call to schedule regular sessions with your therapist!

 

Choosing to work with Physician Counselling Team

STEP 1: Fill in our Intake Forms (Form A – Counselling Request Form and Form B – Confidentiality and Electronic Communications Consent Form)

Form A allows you to select your preferences for therapy format (online or in-person), type (individual, family or couples), language (English, Mandarin or Cantonese) and a preferred physician (each physician profile is available in the ‘About Us’ section on the website detailing their education, experiences and modalities practiced)

STEP 2: Complete the “Getting to Know You Form” and obtain your referral from family doctor

Once we have received both your intake forms, our office will send you the third intake form called the “Getting to Know You Form”. We will also require you to obtain a referral letter from your family doctor to proceed to booking appointments with our physician.

STEP 3: Book your initial consultation
Our office will contact you once the designated physician is available to have the initial consultation with you. During the initial consultation, the physician will assess your current concerns and therapy goals. Our office will notify you once you have been accepted as a formal patient.

Before Your Session

Two minds connected in psychotherapy

Once you have your first appointment booked, you may be requested to fill in a series of in-depth, evidence-based psychological questionnaires to help your physician/therapists understand your concerns and emotions better.

Initial Sessions

During the first session we will discuss your presenting concerns, strengths, resources for recovery, present family situation, personal history, relationships, and family background. By the end of the second session, we will usually have a sense of the issues that need to be addressed, their causes, and some strategies to resolve them. If you choose to begin working through these issues, your therapist/family physician will work with you to create specific goals and decide on the number/frequency of subsequent counselling sessions. Those subsequent sessions will focus on implementing the discussed strategies to address your concerns.

Virtual and In-Person Counselling

Counselling sessions are conducted via secure online video-conferencing service or phone call and usually last for 45-120 minutes, as discussed with your therapist/family physician. With your consent, sessions may be attended/conducted by qualified interns under the supervision of the therapist/physician. 

In-person counselling is available for selected physicians and therapists at our clinic.

Available therapists offering virtual sessions:

  • All Grace Health Centre therapists

Note: If you are requesting for a couples or family therapy, you will only need to fill out Form A once for your family as the main registrant. Each member joining therapy will need to fill out Form B separately.
